Medical Views on Renal Disease – Hypertension

Monitor your hypertensive patient with renal disease for fluid overload. Assess his fluid status by taking his daily weights, checking his fluid intake and output and breath sounds, and checking him for peripheral edema. Maintain fluid restrictions, as ordered, and assess his serum electrolyte levels for hyperkalemia, hyperphosphatemia, hypocalcemia, and hyponatremia. Administer a calcium-based phosphate binder, as necessary. And restrict has potassium intake, as ordered.

Monitor your patient’s hemoglobin level and hematocrit for anemia. If the physician prescribes erythropoietin for anemia, administer it I. V or subcutaneousiy, as ordered. Because the drug may worsen your patient’s hypertension, monitor his blood pressure closely.

If the physician prescribes an antihypertensive drug, administer it, as ordered. Monitor the drug’s effectiveness by measuring your patient’s blood pressure frequently. For a hospitalized patient, expect to measure it every 4 hours for the first 24 hours. For an outpatient, plan to measure it two or three times a day for the first day or two.

Cautiously administer drugs excreted by the kidneys. Such drugs include digitalis glycosides, aminoglycosides, penicillin, tetracyclines, and narcotics. If the physician has prescribed any of these drugs, he may have to modify your patient’s dosage. Do not administer meperidine to your hypertensive patient with renal disease because its metabolite is cleared by the kidneys and can cause seizures as it accumulates.

If the physician has prescribed dietary restrictions, help your patient design a diet plan for maintaining the restrictions or refer him to a dietitian, as needed. If the physician has prescribed a fluid restriction, tell your patient to comply with the restriction by calculating his intake and output.

If the physician has prescribed a calcium-based phosphate binder, tell your patient not to take iron because aluminum and calcium bind the iron. Instruct your patient on self-care measures, such as taking his daily weights and measuring his blood pressure. Teach him to identify edema and electrolyte imbalances, such as hyperkalemia. Tell him to report either of these to his physician.

If your patient will have a home care nurse, tell him that she’ll perform physical assessments, including measuring his daily weights and urine output, checking his breath sounds, and checking for edema. She’ll also assess him for signs and symptoms of electrolyte imbalances and evaluate his compliance with the drug regimen and dietary restrictions.

Tell the patient that she’ll provide assistance as he adapts to his illness and its accompanying restrictions. She may suggest support groups and counseling, if needed. She’ll provide emotional support and encourage him and his family to participate actively in the treatment program.

In 2007, approximately 38,000 people entered some form of substance abuse treatment in Washington State. Approximately 32,000 people of the people admitted were admitted for some sort of drug abuse. However, cocaine is still available throughout Washington and is usually smuggled into Washington from Mexico, through California. Within Washington, crack cocaine is most commonly used in lower income urban areas while snorted cocaine is consumed by a more middle-class demographics.

In the state of Washington, meth / methamphetamine use is well above national averages. 6,378 people (16.7%) were admitted into facilities for amphetamine abuse, over twice the national average of 7.7%. Amphetamine abuse is still not as common in Washington as neighboring Oregon or California, but it is a relatively large problem within the state. Interestingly, since 2003 meth lab incidents in Washington have declined from 1,018 in 2003 to 122 in 2007. This indicates law enforcement’s crackdown on domestic meth labs. As local meth labs have been shut down, most of the drugs have been supplied by meth traffickers from Mexico. Meth also tends to be less of an urban drug, with most of the meth userbase located outside of major cities.

The development of a human being is a traumatic or pleasurable journey, depending on the reception a female egg receives on its journey from ovary to uterus via the fallopian tube. The follicle then forms into a cyst, corpus luteum, responsible for producing progesterone to nurture a fertilized egg. This stage signals PMS symptoms such as breast soreness, moodiness and water retention. The final stage is when this fertilized egg embeds itself in the uterine wall and produces human Chorionic Gonadotropin (hCG). The presence of this hormone is instrumental in alerting the ovary to continue with its function. If somehow the ovary does not receive the signal it stops generating progesterone 9 to 11 days after ovulation, causing shedding of uterine lining, or menstruation, and is the start of another Ovulation Cycle.

The crucial difference in individual Ovulation Cycles is how women accept this most fertile period of the cycle. The egg can live for about 20 hours after ovulation, and if 2 eggs get matured the second is released within 24 hours of the first. It is a possibility that an Ovulation Cycle can occur without ovulating. It generally occurs in girls who have just started menstruating, breastfeeding women, or those going through menopause, or in women whose cycles extend to more than 35 days.

There is a flip side to every pleasure. The Ovulation Cycle can be a cause of ovarian cancer, due to its demand of rebuilding of tissues and new cells through synthesis of DNA.
